LIFE COACHING SERVICE AGREEMENT
The following represents the Agreement between Yolanda Moore (“Life Coach” or “Coach”), who is the representative of Yolanda Moore Global LLC, and the “Client”. The Services contracted shall be that of life coaching. Please read it carefully and raise any questions or concerns before agreeing to the contract. This agreement will begin upon acceptance by the Client and shall remain valid for the duration of the Client-Coach relationship as stated by the package chosen by the Client.
Professional Advice
Please note that life coaching should not be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, or substance abuse treatment and the Coach will not use it in place of any form of diagnosis, treatment, or therapy. Coaching is not to be used in lieu of licensed professional advice.
The Client understands that the Coach is not a licensed professional and agrees to seek professional guidance for legal, medical, psychological, financial, business, or other matters as needed and understands that all decisions in these areas are his/her sole responsibility.
Services
The services provided are generally conducted via telephone or Zoom. Coaching is a partnership focused on developing the Client’s awareness, perspective, and abilities in order to help the Client identify and achieve his/her personal goals. In coaching conversations, the coach will be direct and honest and encourage the Client to do the same. The success of the coaching engagement depends upon the Client’s commitment and openness to the process. If the Client believes that coaching is not working as desired, the Client agrees to communicate this to the Coach.
Terms of Coaching
Receiving life coaching is a process. Many people create change for themselves in a short time. However, to refine and sustain the change takes several months. Although not binding, it is recommended and expected to maintain the service for at least 3 months.
Coaching Fees
Coaching fees are paid in advance of sessions and are outline in the package. Payment for Coaching Sessions is to be made online through automation. In the event Client fails to pay any amount due by 24 hours prior to the scheduled session, it may be cancelled at the Coach’s discretion.
Coaching sessions are typically one hour in length. The introductory meeting may be provided at no charge. This gives the Coach and Client a chance to get to know one another and decide if a comfort level can be established. After the introductory meeting, services provided by the Coach will be performed at the rate specified in the package of the Client’s choosing.
Sessions are conducted via zoom or by phone and check-ins will be conducted by phone, email, or text, whatever is specified in the Client’s package. After the coaching has been completed, we will evaluate together. Additional sessions for monitoring are typically suggested for up to one year or more, on a case-by-case basis.
Waiver
The Client acknowledges and agrees that in the course of the coaching services, he/she may be asked personal, challenging, or disturbing questions. The Client also acknowledges and agree that he/she are fully responsible for his/her physical, mental, and emotional well‐being, and for any actions, choices and decisions made as a result of the coaching. The Client waives and releases the Coach of any claims arising as a result of the Coach’s questions, suggestions, and advice (or lack thereof) and from actions, choices, and decisions he/she may make.
Confidentiality
The Coach will fully protect the confidentiality of communications and records and will only release information about our work with the Client’s written authorization or unless required to do so by law. Coach is bound by the International Coaching Federation (ICF) Code of Ethics.
As it is known some means of communication, such as wireless telephones and e‐mail may not be secure from eavesdropping or hacking. It is impossible to guarantee the protection of confidential information in certain circumstances involving computers and other electronics. This is particularly true of email, text and information stored on computers that are connected to the internet. The Coach will take measures to prevent unauthorized access to information by utilizing encryption and other forms of security protection. The Client acknowledges that the Coach cannot be held responsible for instances of loss of confidentiality through wireless telephone or computer hacking.
Exceptions
There are specific situations in which the Coach deems ethically obligated to breach confidentiality in order to protect the Client or others from harm. If the Coach has information that indicates that the Client, or anyone in his/her care is being abused or in imminent risk of danger or crime, the Coach will need to take protective actions. These situations are quite rare in coaching practices. If such a situation occurs in the relationship, the Coach will make every effort to discuss it with the Client, before taking any action.
Non-Disclosure Agreement
The Client WILL NOT share any of this agreement content, course/session content, course/session contacts, that is provided by the Coach, at any time either written, verbally, electronically, or physically with anyone. The Client understands the information that is shared with him/her is CONFINDENTIAL and WILL NOT share it in any way with anyone. The Client also understands that the information shared in these sessions is meant to assist with his/her personal concerns but is NOT A GUARANTEE of his/her success. Success of this program will be based solely on the Client’s ability to use the tips and strategies that are taught within the sessions. The Client understands that these sessions are not designed to serve as a replacement for licensed professional advice or counseling.
Termination of Agreement
This agreement will remain in effect for the term listed in the Client’s package or until terminated by either party. If either party wishes to end this Coaching Agreement, he or she will notify the other party either by phone or email.
Cancellations/Refund
The Client must give 24hrs notice if he/she needs to cancel or change the time of an appointment. Cancellation (less than 24hrs before) and/or no‐show appointments will be recorded as completed sessions and are not refundable.
The Client may cancel an order within 24 hours of purchasing and request a refund. A refund will be issued, minus a 15% transaction fee. To cancel an order, send your request help@yolandamoore.com. The refund will be issued through the method in which it was received.
If the Client cancels after beginning sessions and/or receiving services with Yolanda Moore Global, they may only request a refund for a prorated amount pertaining to remaining sessions or hours not yet logged. The Client will not have the right to cancel or request a refund after the services have been fully performed and/or after all sessions or services have been completed. A 15% transaction fee of the refund amount will apply and be subtracted from all refunds.
Client Responsibility
This Contract constitutes the entire agreement between the Client and the Life Coach. The Coach shall not be held responsible and shall remain blameless for any adverse or other consequences, including but not limited to financial, personal, employment or other losses, arising out of decisions or actions the Client may make as a result of the Services provided by the Life Coach. The Client understands that the Coach is not a licensed professional and cannot guarantee a specific outcome. The Client understands that the Coach only gives advice in which it is solely on the Client to decide whether to act upon. The Client assumes all responsibilities resulting from his/her own actions.
